What is up with the old lady at the end of the exorcist?

i was watching the exorcist and near the end of the movie the younger priest walks into the room with the girl and sees a freaky old lady sitting up in her bed. who the crap is the lady and y the crap is she there and Y DOES SHE LOOK SO CREEPY
